Product Information

 

Metfor is used for the treatment of type 2 diabetes mellitus, when diet and exercise are not enough to control blood glucose levels. This drug contains 500mg of the active ingredient Metformin hydrochloride. Type 2 diabetes mellitus is a form of diabetes characterized by having high blood sugar. Either the body doesn’t produce enough insulin or the body produces insulin but does not react properly to the hormone. The hormone insulin is responsible for converting the glucose from the food intake into energy.  Metfor may be used alone or with other antidiabetic drugs like sulphonylureas and insulin, together with proper diet and exercise, in managing diabetes. Metfor is also used in treating Polycystic Ovary Syndrome (PCOS).

 

Metformin improves glucose tolerance in patients with diabetes by decreasing intestinal absorption of glucose, decreasing glucose production by the liver, and improves sensitivity to insulin by increasing insulin uptake and utilization in muscles and adipose tissues.

Dosage

 

The recommended initial adult dose of Metfor is 500mg orally twice daily or 850 mg orally once daily. The dose is then increased to 500mg weekly or 850mg every 2 weeks as tolerated and based on the response of blood glucose levels. The maintenance dose is 2000mg daily and the maximum daily dose is 2550mg daily in 3 divided doses. The dose is gradually increased to reduce gastrointestinal side effects.

 

The recommended initial pediatrics dose for children aged 10years and above, is 500mg orally twice daily. The dose may also be gradually increased to 500mg weekly. Maintenance dose can reach up to the maximum dose of 2000mg daily in divided doses.

 

For the treatment of PCOS, the recommended dose is 1000mg twice daily.

 

Metfor tablets are taken by mouth, swallowed whole and taken with meals.

 

Note that the above dosages serve as guidelines only. Not all diabetes patients have the same dosage. A physician must be consulted to get the right dosage. 

 

Contraindications

 

Patients allergic to the active ingredient Metformin hydrochloride, all inactive ingredients of the tablet, and allergy to any biguanides must not take Metfor.

 

Metfor is also contraindicated to patients with severe kidney problems and with a history or in a current state of diabetic ketoacidosis.

 

Side effects

 

Metfor may cause common side effects such as mild nausea, vomiting, upset stomach or diarrhea.

 

Metformin is known to rarely cause a fatal condition called lactic acidosis. Seek medical help for the following mild symptoms that have tendencies to get worse over time:

  • Weakness or feeling tired
  • Unusual muscle pain
  • Numbness or cold feeling in arms and legs
  • Breathing difficulty
  • Abnormal heart rate
  • Stomach discomfort
  • Light-headedness, dizziness

 

Patients with reduced liver and kidney function, conditions that cause low blood oxygen levels like congestive heart failure, recent heart attack or stroke, patients with severe acute illnesses, serious infection, dehydration, and patients that consume large amounts of alcohol are at risk of experiencing lactic acidosis.

 

Low blood sugar (hypoglycemia) can occur in patients taking Metformin when calorie intake is inadequate, calorie supplementation is inadequate after a vigorous exercise, concomitant use with other glucose lowering drugs and alcohol consumption. Symptoms of low blood sugar include sudden sweating, hunger, fast heartbeat, blurred vision, dizziness, shaking, or tingling of hands and feet.

 

Metfor use must be stopped and emergency medical help must be sought for signs of an allergic reaction to the drug. Symptoms include hives, trouble breathing, swelling of face, lips, tongue or throat.

 

Warnings and Precautions

 

A healthcare professional must be informed of the following before taking Metfor, in order to determine if the drug is safe for use:

  • Kidney disease
  • Liver disease
  • History of heart disease
  • Recent heart attack
  • Over 80years of age with no recent check-up of kidney function
  • Have taken chlorpropamide

 

Reduced vitamin B12 and folic acid absorption is observed in some patients taking metformin. A physician may recommend regular monitoring if Vitamin B12 and folic acid levels and intake of extra Vitamin B12 and folic acid supplements.

 

Metformin therapy must be stopped 2 days prior to an x-ray of CT scan using a contrast dye injected to veins.

 

Metfor can also interact with other medications and can cause unwanted effects. A physician must be informed of the following medications:

  • Digoxin
  • Furosemide

Hyperglycemia (high blood sugar) may occur if Metformin is taken with drugs that raise blood sugar levels, like:

  • Phenytoin
  • Birth control pills
  • Hormone replacement therapy
  • Diuretics
  • Heart of blood pressure drugs
  • Niacin
  • Phenothiazines
  • Steroid medications
  • Thyroid medications
  • Diet pills
  • Asthma or allergy medications

 

Metformin is under Pregnancy Category C. It should only be used if advised by a physician after weighing benefits over risks. Insulin is the recommended drug of use in pregnant women for maintaining blood glucose levels. Metformin is excreted in breast milk. Caution is advised to breastfeeding women. This drug also promotes ovulation and increases risk of becoming pregnant.

 

Metfor and Alcohol

 

Alcohol consumption while taking Metformin (Metfor) is not recommended as it will increase the risk of the fatal condition called lactic acidosis and alcohol further lowers blood sugar.
