Product Information

 

Tretiva 40mg contains 40mg of Isotretinoin. This drug is a retinoid, or a form of vitamin A that is used to treat severe cystic acne or nodular acne that do not respond to other oral or topical treatment like benzoyl peroxide, clindamycin, minocycline, or tetracycline.

 

Nodules are large, inflamed, hard and painful acne lesions lodged on the deep layers of the skin. Cysts also reside under the skin’s surface but are filled with pus and are softer. Nodules and cysts can exist independently or together as nodulocystic acne. Nodules are formed due to oil buildup, dead skin cells, and bacteria in pore blockages deep in the skin. On the other hand, acne cysts are formed when comedones (whiteheads or blackheads) get infected and ruptures then the immune system responds by producing pus.

 

Isotretinoin works by reducing skin oil or sebum production, shrinks sebaceous glands, and changing skin oil characteristics. It also prevents abnormal skin hardening by stabilizing keratinization process. Isotretinoin also prevents comedone formation and acne inflammation.

Dosage

 

Tretiva 40mg must be taken exactly as prescribed. A capsule of Tretiva must be swallowed whole, as quickly as possible with a full glass of water so that the capsule will not melt in the esophagus and cause irritation. Isotretinoin must be taken with food or milk. A patient must continue taking the medication for the entire duration of therapy. The acne may get worse at first but will improve after some time.

 

Dosage will vary per patient. Below are the usual doses of Isotretinoin:

 

Adult and children 12years and older for Acne – dose is based on body weight determined by a doctor

  • Maintenance dose 0.5mg to 1mg per kilogram body weight per day divided in 2 doses.
  • Maximum dose of up to 2mg/kg per day
  • Duration of treatment up to 20 weeks

 

If a dose is forgotten, it must be taken as soon as remembered but skipped if it’s almost time for the next dose. Dose must not be doubled just to make up for the missed dose.

                                                                                                            

Contraindications

 

Isofair must NOT be used if any of the following apply:

  • Allergic to any ingredient of Isofair or to vitamin A
  • Pregnant, may be pregnant or planning to get pregnant
  • Breastfeeding
  • Taking tetracycline antibiotics or vitamin A-type medications

 

Side effects

 

All medications may cause side effects but not all people will have the same experiences with the medication. Isofair may cause these common and less serious side effects:

  • Chapped or dry lips,
  • dry eyes or mouth,
  • dry, cracking or peeling skin, itching and rash
  • changes in fingernails or toenails
  • dry nose that may lead to nosebleed
  • headache, dizziness, drowsiness
  • Joint or back pain
  • Temporary worsening of acne

 

The use of Isofair must be stopped and a doctor must be notified if any of these serious side effects occur:

  • Seizure
  • Abnormal hair growth
  • Severe blistering, peeling, rash and redness of skin
  • Joint stiffness, bone pain or fracture
  • Unusual bruising or bleeding
  • Fast heart rate
  • depression, sleep problems, aggression or agitation, hallucination, behavioral changes, thoughts or suicide or harming one’s self
  • symptoms of increased pressure in brain – vision changes, sudden and severe headache, pain behind eyes, nausea and vomiting
  • symptoms of infection – fever, chills, sore throat
  • symptoms of stroke – confusion, one sided weakness, slurred speech
  • symptoms of pancreatitis – severe stomach or back pain with or without nausea or vomiting
  • symptoms of stomach or bowel problems – stomach or bowel pain, rectal bleeding, severe and persistent diarrhea
  • symptoms of severe allergic reaction – hives, itching, rash, breathing difficulty, swelling of face, lips, mouth, or tongue

Warnings and Precautions

 

Before using Isofair, it is important to inform a doctor if the patient have any of these conditions:

  • pregnant, planning to get pregnant, or breastfeeding
  • unable to use 2 effective forms of birth control or unable to avoid sexual intercourse
  • personal or family history of heart disease and high cholesterol, bone problems, gastrointestinal disorders, diabetes, asthma, eating disorder, liver disease, hearing problems, pancreas problems, irregular menstrual period
  • personal or family history of mental or mood problems, suicidal thoughts or actions

 

Other medications may interact with Isofair and cause undesirable effects. A patient must inform a doctor of other medications being taken such as, but not limited to:

  • tetracyclines – the risk of increased brain pressure may be increased
  • Vitamin A-type medications – the risk of Isotretinoin side effects may be increased
  • Progestin-only birth control – Isotretinoin may reduce its effectiveness
  • St. John’s wort – risk of hormonal contraceptive failure may be increased
  • Seizure medications
  • Steroids

Isotretinoin must NEVER be used during pregnancy. The drug can cause serious birth defects, miscarriage, early birth and even death to the fetus. Women must have a negative result on pregnancy test 30 days before using Isotretinoin. Women must avoid getting pregnant while taking this medication.  Women must agree to use two forms (primary and secondary form) of birth control 30 days before and 30 days after use of Isotretinoin.

 

Primary forms of birth control:

  • Tubal ligation
  • Vasectomy of male partner
  • IUD
  • Estrogen-containing birth control pills
  • Hormonal birth control patches, injections, implants, or vaginal ring

Secondary forms of birth control:

  • Male latex condom with spermicidal gel or foam
  • Diaphragm with spermicidal gel or foam
  • Cervical cap with spermicidal gel or foam
  • Vaginal sponge with spermicide

 

Isotretinoin use must be stopped and a doctor immediately notified if the patient performs unprotected sex, stopped using birth control, period is late, and might be pregnant.

 

Normal growth of children may be affected if Isotretinoin is used. Caution is advised when used on children as they may be more sensitive to the effects of the drug. The drug’s safety and efficacy on children below 12 years have not been established.

 

Acne condition may worsen and skin may become irritated during the first 3 weeks of therapy with Isotretinoin. Skin condition should improve within 1 to 2 months of using the medication. Otherwise, contact a healthcare provider.

 

Use protective clothing and sunscreen with at least SPF15 regularly to help Isotretinoin to work properly.

 

Isotretinoin may cause drowsiness, dizziness and sudden decrease in night vision. Caution is advised when driving and performing unsafe tasks. It is best to avoid night driving.

 

Things to avoid while taking Tretiva 40mg

  • Do not donate blood while using Isotretinoin and at least 30 days after therapy discontinuation
  • Do not have dermabrasion, laser skin treatments, nor use wax hair removers while using Isotretinoin and at least 6 months after therapy discontinuation for it may cause scarring
  • Avoid overexposure to sun, artificial UV rays, wind, or cold weather. Isotretinoin can make the skin more sensitive to sunlight, sunburn, dryness, and irritation

 

Tretiva 40mg and Alcohol

 

Alcohol consumption may worsen the drowsiness and dizziness that Isofair may cause. Avoid drinking alcohol when taking Isotretinoin.
